How do you apply for a Birth Certificate ?

I beg to state that my daughter Disha Roy was born four years ago at Jirat and her birth was registered at that time in your office. For a number of reasons I could not apply to you for issuing the certificate registering her birth. Now the certificate is necessary for her admission to a school .
I shall be, therefore, highly obliged if you kindly issue a birth certificate in favour of my daughter.
